FAQ

Yes, if you or anyone you’re bringing are to see a doctor and haven’t been registered at the clinic before, you will have to fill out and return a New Patient Form to the front desk.

Medicare rebatable telehealth appointments are available for existing patients only (seen face to face by one of our doctors in the last 12 months). Please ring the practice on 9418 3722 for further information.

Please bring your Medicare card and if relevant, any healthcare or concession cards so we can register you appropriately.

Best practice is to call Cockburn Medical Centre on 9418 3722 as soon as you need to cancel. Please note that a minimum of three hours is needed to avoid a late-cancellation fee.

Prescriptions

Please make an appointment for a repeat prescription if you have not seen your Doctor for three months or more. The doctors have a legal obligation to review your medical condition prior to writing a prescription. Most prescriptions provide sufficient medication to last until your condition needs to be reviewed by your Doctor. if you request a copy of lost prescriptions (or repeats) without seeing the doctor a $10 administration fee applies. 

Referrals and other requests

Please make an appointment with your doctor to get a referral. This allows the doctor to ensure the specialist gets your most up-to-date medical information and is also our legal obligation under Medicare rules. Copies or renewals of referrals, certificates, faxing requests etc, incur a $10 administration fee to be paid before the request is fulfilled.
